Updated Occupational Health referral form

Our Employee Assistance Provider PAM Assist is now able to offer the option of face-to-face appointments with their Occupational Health Advisor. These will take place at 45 John Street from Wednesday to Friday each week, with a maximum of five appointments per day.

We have updated our Occupational Health referral form to reflect this change. The updated form is available under the Occupational Health header on the  Attendance Management Support page of the intranet. Here you can also find our Occupational Health guide for managers.
