Live Well Community Referral

What is Live Well?

Live Well is a free programme helping people to lead better lives and is for those who need support accessing our activities and services. Whether that is joining community clubs, becoming more active or making new friends. Trying something new can be scary and that is why we are here to help.

Notification

Live Well Participants have said:

"I have met some wonderful people, had fun and improved my health”

"They’re always willing to share stuff: recommendations, resources."

Our Live Well team has built strong relationships with local groups. This allows us to link people with activities which are happening in their area. Being able to signpost someone to an activity they are interested in is incredibly valuable.

Live Well gives people encouragement to join activities in their community. We know people often need a bit of support to have the confidence to get involved. Whether that is joining a walking group to get active and making new friends, taking part in a volunteer session or learning something new.

You will be linked with one of our advisers when you take part in Live Well. They will help you to access these activities and programmes in your area.

How can Live Well help you?

We help people to take part in sport sessions delivered by Glasgow Life. We also connect people to museum activities, arts and craft sessions, singing groups and local physical activity classes.

Making a referral

Who can refer?

We welcome referrals from anyone, including health professionals and housing. Social work, Police Scotland and community groups can also make referrals. All we need to know is that you have a person's permission to provide us with their details. They should also be expecting us to contact them.

Who can be referred?

Live Well will link people to activities across Glasgow. It is open to anyone aged 18 and over who either lives in or is willing to travel to project areas. The benefits for those taking part include:

  • becoming more connected with their community
  • meeting new friends
  • improving their mental wellbeing
  • increasing their physical activity levels
  • learning new skills, volunteering or finding wellbeing information

Where is support offered?

Our team will link people to activities across Glasgow and can offer personalised support in the following areas of the city:

  • North - Milton, Possilpark, Springburn
  • West - Drumchapel, Knightswood, Yoker
  • East - Barlanark, Bridgeton, Carntyne, Easterhouse, Parkhead

Referral progress

We can update you on the progress of your referral. To do this, please let us know when you make the referral and make sure you have the participant's permission.

We may also contact you to get feedback about your experience. Your comments help us to develop a person-centred approach to our community wellbeing programmes.

What to expect

Once you have made the first step to take part in Live Well, our friendly team will be in touch to chat with you.

Next steps

Our team will be in touch within three days if you have been referred or asked to be contacted.

You can also speak to one of the team on freephone 0808 175 1956 between 9am-5pm Monday to Friday.

Welcome chat

When we first contact you, we'll chat about your wellbeing and listen to what level of support you feel you may need. This is so we can connect you to local activities and groups.

The team is locally based and will:

  • listen to what matters most to you
  • help you find new activities you can try or rediscover things you used to love doing
  • help you to feel more confident about going to activities and new places
  • enjoy a cup of tea and a chat with you

What activities can you get involved in?

Our team will help you find and take part in local activities including:

  • art and creative activities
  • social groups
  • museum activities
  • walking groups
  • exercise classes and sports
  • family activities
  • computer basics
  • learning and skills classes
  • volunteering

For an idea of the kinds of sessions on offer, take a look at our activity finder.

We can also connect you with other services if you have any other issues affecting your wellbeing. Our team will work with you to find the support you need.

How long does Live Well last for?

We will aim to support you for up to 12 weeks. This depends on how comfortable you feel using Glasgow Life services and continuing to use them on your own. It may be for a shorter time if you feel you don't need our support anymore.

We will always be here if you need us again in the future.

Study shows positive impact of Live Well

An independent evaluation of the Live Well Community Referral programme has shown it is helping Glaswegians to live healthier and happier lives.

Many of the people we work with want to increase their physical activity. Others are looking to connect with the local community to combat loneliness.
